If you are going to judge whether anything is working — a diet, a routine, a supplement — you need to know what you are measuring. These three numbers tell you different things over different timescales, and confusing them is the most common mistake people make.
A single measurement taken after at least eight hours without food, usually first thing in the morning.
It reflects how well your body maintains glucose overnight without any dietary input — largely a question of how much glucose your liver is releasing and how effectively insulin is restraining it. Elevated fasting glucose often indicates insulin resistance at the liver.
Its limitation is that it is one moment. Fasting glucose varies day to day with sleep, stress, illness and what you ate the previous evening, so a single reading tells you less than people assume.
The rise in glucose after eating, typically peaking somewhere between thirty and ninety minutes depending on the meal.
Everyone experiences these — a glucose rise after eating carbohydrate is normal physiology, not a malfunction. What matters is the height of the peak and how quickly it returns to baseline. Higher and longer excursions indicate the system is struggling.
Post-meal glucose is where the most actionable interventions apply: meal composition, sequencing, portion size and walking afterwards all affect this specifically. It is also the measure most people never see, because routine testing is usually fasting.
The most important of the three for long-term assessment, and the most misunderstood.
Glucose in the bloodstream binds irreversibly to haemoglobin in red blood cells. The higher your average glucose, the higher the proportion of haemoglobin that is glycated. Because red blood cells live roughly three months, HbA1c reflects average glucose over approximately the preceding two to three months, weighted toward the more recent weeks.
This is why it is useful: it cannot be gamed by fasting before the test, and it smooths out daily variation.
It is also why no intervention can change your HbA1c in thirty days. If a product promises a meaningful HbA1c change within a month, that claim is incompatible with how the measure works. The red blood cells carrying the old average are still in circulation.
People are often puzzled when their fasting glucose looks acceptable but their HbA1c is elevated, or the reverse.
The usual explanation is post-meal excursions. Someone can have normal fasting glucose and still spend several hours a day at high levels after meals, which raises the average that HbA1c captures while leaving the morning reading looking fine.
There are also conditions where HbA1c misleads: anaemia, recent blood loss, some haemoglobin variants, kidney disease and pregnancy all affect red blood cell lifespan and can distort the result in either direction. This is worth mentioning to your doctor if your numbers seem inconsistent.
Diagnostic thresholds vary somewhat between countries and guidelines, and interpretation belongs with your doctor rather than a website. Broadly, in widely used criteria:
Two notes. A diagnosis is not made on a single reading — it requires confirmation. And these are thresholds on a continuum, not cliff edges: risk rises gradually rather than switching on at a particular number. The American Diabetes Association publishes current guidance.
CGMs have become widely available to people without diabetes, and they change what is visible. Instead of one fasting number, you see the full curve — which meals spike you, how long you stay elevated, what overnight looks like.
Two honest caveats. Seeing normal physiological glucose rises can cause unnecessary anxiety, because a spike after eating is not pathology. And there is a risk of over-optimising individual meals while ignoring the larger factors — weight, sleep, activity — that actually determine long-term control.
Used sensibly, though, a few weeks of CGM data teaches you more about your own responses than any general article can, including this one.
